Biotin supplementation interference in health testing

How much do you know about Biotin? Commonly known as Vitamin B7, Biotin is taken by as many as 20% of people, including high profile celebrities like Kylie Jenner, Kim Kardashian and Vanessa Hudgens, for its hair and nail-strengthening properties.

If you take biotin supplements, it is important to know the effects it can have on particular blood tests, should you visit your GP or local hospital.

Whilst there are various diagnostic health tests out there, including fertility hormone tests, prostate tests, and tests for troponin – a marker released into the blood during a heart attack – that are known to be affected by elevated levels of biotin, there are other tests available, including those provided by Randox, that are not impacted.

The reason that so many non-Randox tests are impacted by biotin is that biotin is widely used throughout the biotechnology industry in the development of diagnostic tests.

ELISA tests in particular (tests that measure the reaction of antibodies to identify a substance) often make use of antibodies labelled with biotin, to detect toxins or other foreign substances within the body.

In most instances, the biotin will bind to a protein called streptavidin. This bonding is the strongest non-covalent biological interaction known, and is therefore particularly useful in binding antibodies within diagnostic tests.

But with more and more people exceeding the recommended daily dosage for biotin (30 micrograms) by taking up to 10,000 micrograms of biotin in supplements marketed for beauty reasons, many diagnostic tests are being measured inaccurately. Excess biotin in the blood can block the binding of biotin-labelled antibodies to streptavidin within the tests, and the substance being tested won’t be measured accurately.

Laboratory professionals have known about this potential problem for some time. In late November 2017, the FDA (Food and Drug Administration) published a safety notice to make the public and healthcare practitioners more aware that biotin can “significantly interfere with certain lab tests and cause incorrect test results.”

The FDA even reported on one particular case in which a patient died following falsely low troponin (marker of a heart attack) results when the troponin test used was known to have biotin interference. Biotin supplements masked the true diagnosis of a heart attack.

Many patients taking biotin supplements have also been misdiagnosed with a condition called Graves’ disease, an autoimmune condition characterised by an overactive thyroid. In these cases, biotin supplementation led to falsely low levels of thyroid stimulating hormones, and falsely high levels of other thyroid hormones. This particular profile of hormones led to a Graves’ disease diagnosis, in spite of a lack of symptoms, which in Graves’ disease would usually include muscle weakness, a quickened heartbeat, sleeping problems, diarrhoea, weight loss and poor tolerance of heat.

In pregnant women, tests that are impacted by biotin interference may produce falsely low results of beta HCG, more commonly known as the ‘pregnancy hormone’ as it released by the placenta after conception. With low levels of beta HCG, and therefore no confirmation of pregnancy, pregnant women could be exposed to X-rays and CT scans that may harm the developing foetus.

So, what can be done? Suggestions have been made that patients taking biotin supplements should be made to wait before any diagnostic testing is conducted, so that the biotin clears from their system.

But this “Wait and Watch” approach certainly would not work in emergencies. In the case of a heart attack, testing must be conducted as soon as possible to allow for diagnosis, immediate medical intervention and follow-up testing.

In the GP setting, general practitioners must ask their patients if they are taking any biotin supplements and inform the testing laboratory if interference from biotin is a possibility. They should also consider that lab results not matching with a patient’s signs and symptoms may be caused by biotin interference.

In the acute care setting however, it is imperative that biotin technology is not used in diagnostic testing to protect patients from misdiagnosis and subsequently, further health problems.

In a record seizure of synthetic drugs for Spain, police have confiscated approximately 827,000 ecstasy tablets, 76kg of speed, 39.5kg of crystal meth and 217 litres of liquid amphetamine at the start of 2021.

Eleven people – from Spain, the Netherlands, Romania, Colombia and Italy – were arrested during a joint investigation by the Guardia Civil and national police forces, according to a statement. The police operation, which lasted nearly a year, resulted in the largest synthetic drugs seizure in Spain, and second-largest ecstasy seizure in Europe over the past 20 years.

The criminal organisation was described as “the most important criminal organisation dedicated to the manufacture and trafficking of synthetic drugs in the history of Spanish drug trafficking”.

Once the large haul was in Spain, the drugs were planned to be processed in two laboratories set up in Barcelona.

“It’s worth noting that of all the substances seized, the 217 litres of liquid amphetamine would – had it been processed in the organisation’s Barcelona laboratories – have produced 738kg of speed, making it the biggest ever amphetamine seizure in Spain,” a statement said.

Dog Food Recalled Due to Potentially Unsafe Levels of Aflatoxin

The importance of screening food for toxins has been highlighted this month with the recent news that Midwestern Pet Foods Inc. have extended their product recall after reports that the pet food has potentially caused the deaths of 70 dogs due to containing possibly unsafe levels of aflatoxin.

The Food and Drug Administration said in a statement that it has been “aware of more than 70 dogs that have died and more than 80 that are sick after eating Sportmix pet food.”

Aflatoxins are mycotoxins produced by two species of Aspergillus, a fungus which is especially found in areas with hot and humid climates. Aflatoxins have a wide occurrence in feed and cereals, such as wheat, barley, rice and corn, which are used as ingredients in pet food. At high levels, aflatoxin can cause illness and death in animals.

Midwestern Pet Foods first announced a voluntary recall on Dec. 30, extending it last week. “There have been reports of illnesses and deaths in dogs associated with certain lots of products. No human illnesses have been reported,” the company said in a recall announcement. “Out of an abundance of caution, we have expanded this recall to cover all corn products containing pet foods with expiration dates prior to 07/09/22.”

Aflatoxin poisoning can display in animals with symptoms such as sluggishness, loss of appetite, vomiting, jaundice and diarrhoea.

The expanded recall includes “additional corn-containing lot codes of Sportmix, Pro Pac Originals, Splash, Sportstrail, and Nunn Better dry dog and cat foods” that were produced in its Oklahoma manufacturing plant. A full list of recalled products can be found here.

India Must Stop Misusing Antibiotics in Dairy Sector, According to CSE Report

The Centre for Science and Environment (CSE) has raised concerns over whether the milk we consume is safe. Hosting a virtual meeting in July, the CSE discussed the use of antibiotics with the world’s largest producer of milk, India. Over 188 million tonnes of milk was produced by India over the period of 2018-2019 and lead with 22% of global milk production.

A recent assessment by the CSE has found indication of antibiotic abuse in milk producing cattle.

“We have found that antibiotics are extensively misused in the dairy sector; antibiotic residues remain largely untested in milk, an integral part of Indian diets, particularly of children,” said CSE Director General Sunita Narain. “While we continue to struggle against COVID-19, we are staring at another pandemic like situation – that of antibiotic resistance fueled by the way we are producing our food, which has become chemical-intensive.”

Antibiotics such as penicillin, aminoglycosides and fluroquinolones are being ‘indiscriminately’ used by dairy farmers according to the CSE. These antibiotics are among the most important in human health. The World Health Organisation (WHO) has warned as we face growing concerns over an antibiotic resistance, such antibiotics should be preserved.

Despite laws stating that antibiotics should only be prescribed and administered by a registered veterinarian, the CSE said that some farmers are illegally attaining and injecting animals themselves without the supervision of veterinarians.

“Farmers often sell milk while the animal is under treatment, which increases the chances of antibiotic residues in the milk,” explained Amit Khurana, the CSE’s Food Safety and Toxins Programme Director. “While milk sold directly to consumers is not tested, contrary to what one would expect, processed milk sold in packets is also largely unchecked for antibiotic residues.

“This explains why, despite pooling and processing, packed milk samples from several states had antibiotic residues in the FSSAI’s [Food Safety and Standards Authority of India] milk quality survey of 2018.”

However, CSE noted the virtual meeting has encouraged positive signs of action in the diary industry in India.

“The issues highlighted by the CSE assessment have been well recognised by the experts and stakeholders. These experts have also recommended several measures for minimising antibiotic misuse in the Indian dairy sector.”

In order to minimise the threat of antibiotic resistance, the CSE have recommended “preventative measures” such as;

  • Routine observation of drugs residues in milk
  • Modifying the existing standards for antibiotic residues in milk
  • Limiting misuse of essential antibiotics such as penicillin

Such measures, Khurana noted, are showing some success. “Information shared by various organisations, agencies and experts suggest that ethno-veterinary medicines, better management of sub-clinical mastitis, and good farm management are contributing towards reducing antibiotic misuse.”

“India is now talking about antibiotic resistance in the one-health perspective,” noted Narain. “There is a shared concern, and all voices are now together in expressing that concern.” As such he believes further (and necessary) action will be taken.

“We must ramp up surveillance and testing, put a complete stop to the use of critically important antibiotics and penalise their use,” he added, “and work with farmers and the agriculture-dairy sectors to innovate on solutions.”

Drug overdose deaths in the USA rose to a record high in 2019, after falling for the first time for 25 years in 2018. Almost 72,000 people died from drug overdoses in 2019 according to the preliminary data that was released by the Centres for Disease Control and Prevention earlier this year.

Though there is currently no hard data available, there is speculation that the pandemic is complicating and worsening the surge, as 2020 looks to be even more devastating than 2019. Data collected by The New York Times saw for the first half of 2020 drug overdose deaths have risen by an average of 13 percent across America compared to last year. If this trend continues into the rest of the year, it will be the sharpest rise in annual drug deaths since 2016, when fentanyl made its way into US drug trafficking and street supply.

Increased funding toward addiction treatment, prevision and recovery services did shows signs of improving overdose deaths. Though the government has made this positive headway in the last few years in how they tackle the drug epidemic in America, this progress could be threatened by the current COVID-19 pandemic.

Earlier in the month, Brett Giroir, the administration’s assistant secretary for health, said in a statement, “We understand that there is an extraordinary amount of work to do, especially now as we are also dealing with the Covid-19 pandemic that could markedly affect our nation’s mental health and risk of substance use.”

Several public health experts said conditions created by the pandemic could hurt the nation’s fragile progress in fighting the surge of drug deaths but noted that the overdose rate was on its way back up well before the virus arrived. “Covid just makes it a bit worse,” said Dr. Dan Ciccarone, a professor of medicine at the University of California, San Francisco, who studies the opioid epidemic. “It’s a small wave riding on top of a tsunami that continues to devastate.”

Although official statistics on 2020 overdose deaths won’t be available for a long time, there is local evidence showing drug related deaths are rising into 2020.

Though a lot of this data precedes the coronavirus pandemic, researchers believe there are several reasons why the virus could worsen the trend.

When state lockdowns first began in March, Dr. Anna Lembke, a clinician with Stanford’s Addiction Medicine Dual Diagnosis Clinic, noticed many of her patients showing signs of improving. “Many patients described a kind of peacefulness without the constant hubbub of modern life and the constant triggers they’re exposed to,” she said.

However, in response to the virus, the US government has done something unheard-of: it relaxed rules and regulations around prescribing methadone and buprenorphine, two commonly prescribed opioid addiction treatments. Patients no longer needed to attend daily in person appointments to receive their methadone prescriptions and could instead be given 4 weeks’ worth at a time. Doctors no longer need to meet in person to prescribe buprenorphine.

This was initially celebrated as a positive change, however as the pandemic has progressed and people have remained isolated, lockdown has become harmful to people with mental health issues and drug addiction disorders.

“Social isolation has always been a huge component of drug overdose risk,” said Traci Green, an epidemiologist at Brown University who studies drug abuse and addiction. “So much of what we’ve been trying to do has been completely unravelled.”

Experts have pointed to other dangers lockdown poses such as;

  • Lack of revival fall back from other users due to an increase in solo drug users
  • Less emotional support as in person visits are scaled back
  • Inconsistent drug strength and quality
  • Supply or income disruption leading to inconsistent drug use habits and accidental overdose

As Covid-19 infection rates continue to rise in many parts of the world, an increasing number of countries are asking for a negative PCR test as a means of proof that travellers do not have the Covid-19 virus.

Whilst not yet a common requirement globally, it is understood that the EU could soon be turning to a Traffic Light System that will require pre-departure testing for individuals departing any of the 15 ‘red list’ countries – which includes the UK and could be extended to include Ireland.

Whilst there are currently a number of countries across the EU that do not require a negative COVID test for visitors from the UK, the implementation of this co-ordinated approach across the EU will place common requirements on all travellers from the UK, no matter which country they are visiting.

It could also be used in support of reduced quarantine in those countries which require it.

So – what is a PCR test?

PCR stands for Polymerase Chain Reaction and is a method of testing used to detect infectious disease, including COVID-19.

A PCR test is performed in a laboratory and is indicative of an individual’s COVID status at the time their sample is taken. It does not show previous infection.

Where can I get a COVID PCR test done?

PCR tests are available at Randox Health via an in-clinic appointment, or a home sample collection.

Are home tests accepted as proof of my COVID status?

It is important to note that many countries do not accept ‘home tests’ as proof of a negative COVID-19 result. Results must show that the test was performed in a certified laboratory.

Your destination country will determine the time frame required for testing. For example, it may state that your sample needs to be taken and tested within 72 hours before departure / arrival. You cannot be swabbed before this time.

It is best to book / order your test in advance and take it as early as possible within your window, so that you allow for the maximum amount of time to get your test results back.

How long do I wait for my results?

    The term ‘cardiovascular disease’ (CVD) refers to any disease of the heart, vascular disease of the brain, or disease of the blood vessel. More people die from CVDs worldwide than from any other cause: over 17.9 million every year, according to the World Health Organization (WHO). Of these deaths, 80% are due to coronary heart diseases (eg heart attack) and cerebrovascular diseases (e.g. strokes) and mostly affect low- and middle-income countries.

    To try and raise awareness of the dangers of CVD and associated conditions, the World Heart Foundation celebrate World Heart Day on September 29th every year to inform and educate on the prevalence of heart disease.

    The UK Situation

    There are around 7.4 million people living with heart and circulatory diseases in the UK. Heart and circulatory diseases cause more than a quarter (27 per cent) of all deaths in the UK; that’s nearly 170,000 deaths each year – an average of 460 people each day or one death every three minutes.

    Healthcare costs relating to heart and circulatory diseases are estimated at £9 billion each year.

    What are cardiovascular diseases?

    Cardiovascular diseases (CVDs) are a group of disorders of the heart and blood vessels and they include:

    • coronary heart disease – disease of the blood vessels supplying the heart muscle;
    • cerebrovascular disease – disease of the blood vessels supplying the brain;
    • peripheral arterial disease – disease of blood vessels supplying the arms and legs;
    • rheumatic heart disease – damage to the heart muscle and heart valves from rheumatic fever, caused by streptococcal bacteria;
    • congenital heart disease – malformations of heart structure existing at birth;
    • deep vein thrombosis and pulmonary embolism – blood clots in the leg veins, which can dislodge and move to the heart and lungs.

    23 September August 2020: Italian Authorities Seize Over $1B Worth of Amphetamines Originating From Syria

    Italian police intercepted and seized 14 tons of amphetamines with a street value of approximately $1.15 billion at the Italian port of Salerno in early July. This huge shipment was suspected to have originated from Syria before being transported to the base of a notorious European crime organisation in Naples.

    ‘Largest seizure of amphetamines in the world’

    In video footage the Italian Financial Guard military force posted to their Twitter account, authorities can be seen using electric saws to cut through 6-foot-high cylinders in shipping containers. The Customs Police Col. Domenico Napolitano called the discovery the largest seizure of amphetamines in the world after finding over 85 million pills inside the cylinders.

    Syria Identified as One of the Largest Producers of Amphetamines in the World
