Clinical Significance
Not only is apo C-III a key regulator in the metabolism of triglycerides, but it is also an independent risk factor for CVD. Not only does apo C-III contribute to CVD through the regulation of triglycerides but also through its direct atherogenic effects, promoting atherosclerosis (fig 1) 1.
Fig. 1. Pleiotropic effects of apo C-III 1
Physiological Significance
Apo C-III is composed of 79 amino acids and has a molecular weight of 8.8kDa. Apo C-III promotes hypertriglyceridemia via several mechanisms. Firstly, like apo C-II, apo C-III also inhibits the activity of LPL, distributing lipid lipolysis. Apo C-III hinders the binding of apolipoprotein B and apolipoprotein E to the hepatic receptors resulting in the delayed catabolism of triglyceride – rich lipoproteins (TRLs). Finally, apo C-III prefers the production and secretion of very low-density lipoproteins (VLDL) in the liver 1.

Superoxide dismutase (SOD) are a group of metalloenzymes, the first line of defence against reactive oxygen species (ROS)-medicated injury. SOD is responsible for catalysing the dismutation of the superoxide anion free radical (O2–) into molecular oxygen and hydrogen peroxide (H2O2). The aim of this process is to reduce the levels of O2– as it damages cells at excessive concentrations 1.
SOD is the most important antioxidant defence against oxidative stress. SOD is a severe anti-inflammatory agent and can prevent precancerous cell changes. Reduced SOD levels have been observed in older generations as SOD levels drop as the body ages and so are more prone to oxidative related diseases. Reduced SOD activity has been identified as having a strong correlation with Alzheimer’s Disease. The overexpression of SOD-2 reduces hippocampal superoxide, preventing memory defects 1. Moreover, SOD has been identified as a potential diagnostic and prognostic marker in patients with gastric cancer 2. SOD has also been linked to other health conditions, including: rheumatoid arthritis, RBC-related disorders, Crohn’s Disease, cystic fibrosis, malignant breast cancer and neuronal apoptosis 1, 3.

Diabetes diagnosis with Randox Reagents
The prevalence of diabetes is steadily increasing across the world, with approximately 422million people worldwide with diabetes and is currently one of the leading causes of death in the world. A diabetes diagnosis comes in three forms; Type 1 Diabetes Mellitus, Type 2 Diabetes Mellitus and Gestational Diabetes. Each type of diabetes can have long-term, detrimental effects to your health if it is not controlled, with some of the key complications being heart disease, kidney damage, retinopathy and even limb amputations.
Diabetes can be controlled through maintaining a healthy diet and regular exercise, however in situations where complications occur, innovative testing can aid in the prevention and management of detrimental consequences to patients. Randox Reagents offer a range of high performance and unique tests which can be used to manage complications of diabetes such as:
Diabetic Nephropathy
Kidney disease is a life threatening complication of diabetes, commonly called diabetic nephropathy in patients with diabetes. Around 40% of people with diabetes develop diabetic nephropathy, characterised through prolonged periods of high glucose levels in the blood. To effectively monitor diabetic nephropathy, it is essential to test cystatin C levels in patients, which is a useful indicator of renal function in patients where creatinine measurements are unreliable. Unlike creatinine, cystatin C does not have a ‘blind area’ – up to 50% of renal function can be lost before significant creatinine elevation occurs. This makes cystatin C capable of detecting early stage kidney dysfunction in patients with diabetic nephropathy.
Microalbumin testing is also important to identify patients with diabetic nephropathy approximately 5-10 years earlier than proteinuria tests, helping to reduce the incidence of end stage renal disease. This is because low albumin concentrations in the urine are the earliest market of renal damage and therefore enable preventative measures to be taken.
Metabolic Syndrome
Metabolic syndrome is a severe complications of uncontrolled diabetes which contains a number of conditions which occur together, increasing your risk of heart disease, stroke and diabetes. Metabolic syndrome can be monitored through measuring Non-Esterified Fatty Acids (NEFA), which are molecules released from triglycerides by the action of the enzyme lipase and are transported in the blood bound to albumin. NEFA contributes a small proportion of the body’s fat, however they provide a large part of its energy, with elevated concentrations having adverse effects on both carbohydrate and lipid metabolism.
With the global burden of diabetes rising year on year, diabetes complications monitoring has never been more important. Randox Reagents offer a wide range of innovative testing to laboratories, to help clinicians accurately diagnose and monitor diabetes complications.

Drugs of Abuse – A Global Pandemic
4th September 2019
Drugs of Abuse – A Global Pandemic
The prevalence of drug addiction and abuse across the globe has become a growing concern. It is currently estimated that 71 million people use drugs of abuse with around 166,000 direct deaths occurring from drug use in 2017.
The WHO (World Health Organisation) estimate that 11 million people inject drugs across the globe, of which 1.3 million are living with HIV, 5.5 million with hepatitis C, and 1 million with both HIV and hepatitis C. In addition to infectious disease, long term drug abuse increases the risk of several cancers, cardiovascular effects, respiratory effects, gastrointestinal effects, kidney & liver damage and mental health issues.
The abuse of drugs is one of the most pressing issues in the USA and other countries globally. Drug abuse not only affects the individual, but can also have far-reaching consequences that affect family, employment, personal health, health care systems, local communities, and society as a whole.
Global Drug Abuse – The buzz that keeps on going!
> Misuse of illicit drugs affects society through secondary costs incurred such as crime, reduced productivity at work, and health care expenses.
> Substance abuse costs the US health care system about $11 billion, with overall costs reaching $193 billion.
> Substance abuse and addiction also affects other areas of life leading to broken families, destroyed careers, death due to negligence or accident, domestic violence, physical abuse and child abuse.
> Drug abuse and addiction changes the chemistry of your brain. The longer an individual abuses drugs, the more damage is caused, making it more difficult to return to ‘normal’ during drug rehabilitation.
> Treating and tackling drug addiction is a challenge. As drug treatment and health services continue to fall short, there is an increased need to continuously adapt drug detection and analysis.
Randox laboratories are at the forefront, aiding the fight against drugs of abuse with a comprehensive range of 10 drugs of abuse assays including several stimulants, sedatives and opioids available to run on Randox’s very own clinical chemistry analysers, the RX series. To find out now how Randox is helping to tackle the ongoing prevalence of drugs of abuse click here or buy directly from the Randox Online Store.
